Our Services
Aging Gracefully offers an abundance of services and amenities designed to meet the needs of our participants.
Our adult day care service is tailored to meet the needs of the caregiver as we provide “A One Stop Shop for Respite Care.” Whether you are going to work, running earrings, or taking that well deserved rest, you will be worry-free with your time away from your loved one. At Aging Gracefully, our staff follows an individual care plan for each participant that is tailored to their needs.
The following is a list of services that are offered within the facility:
- Transportation Services: Our facility van offers transportation to and from the
day care Monday through Friday for ambulatory clients. [Click here to learn more

- Emergency Response System: A call bell system is available in each restroom
stall as well as over each bed in the wellness center for those individuals that
need staff for additional assistance. Our staff is trained yearly through American
Red Cross in CPR and First Aid. In the day care setting, if a client is without
pulse or respiration, the staff will call 911 and provide CPR until emergency
services arrive. If the staff is unable to reach the family in the event of an
emergency and additional staff is available, a staff member will go with the client
to the emergency room until a family member arrives to replace the staff.
- Personal Care Services: Our staff will provide personal care services to each
individual depending on their needs. These services include bathing,
incontinence of bowel and bladder, shaving, assistance with dressing and
undressing, and washing their hands. Each individual careplan is reviewed and
updated routinely in order to provide more advanced services when needed.

- Catered Nutritious Meals and Snacks (Special Diet Plans Available): Our
meals are catered each day through Senior Catering. They provide the facility
with a USDA meal at 12 noon each day. This meal includes a portion of meat,
two vegetables, bread, a dessert, and milk. If a client is a diabetic, the desserts
are substituted with a sugar free item. Supplemental meals can also be given
with a doctors order and if the family provides the supplements. Snacks are
provided at 10am and 3pm each day.
- Supervision by Licensed Nurses: The owner of Aging Gracefully is a
Registered Nurse and is very much involved in the daily services offered. She is
on-call 24/7 to all clients of Aging Gracefully. In the absence of the owner, either
a Registered Nurse or a Licensed Practical Nurse administers the medications
and is on-call for emergencies. [Click here to learn more about the owner.]
- Medical Management: The Nurse will administer medications as scheduled
during the day. Depending on the needs of the client, the registered nurse is
available to provide assistance with medical needs such as foley catheter,
colostomy, wound care dressings, oxygen, and well as nebulizer or inhaler
treatments. All medical treatments, including medications, must have a doctors
order before the registered nurse can provide the service.
- Handicap Accessible Showers: Our staff will also provide bathing services if
requested. Showers may be given on a routine basis and as needed for
incontinence. This is an additional service that increases the clients level of
care.
- Monitoring of Health Status: The staff is trained to monitor for changes in our
client’s health status. The staff routinely monitors for changes in appetite,
restroom habits, changes in behaviors, signs of hypoglycemia/hyperglycemia,
increased weakness, decreased mobility and ability to transfer, nausea/vomiting,
fever, etc. If requested by the primary care physician, the staff will document
and maintain records of daily blood pressures for clients on blood pressure
medication or blood sugar results for diabetics. This information is documented
and communicated with family and/or physician as needed.
- Therapeutic Activities: The activities department schedules and implements a
wide range of activities throughout each day to stimulate the client’s and
encourage participation, memory stimulation, and fun-filled laughter. Some of
our activities include daily exercise, crafts, bingo, card games, board games,
puzzles, music therapy, pet therapy, community involvement (such as child day
cares/schools, local churches, and volunteers that visit to sing, dance, play the
piano, organ, or guitar. We are partnered with the Retired Senior Volunteer
Program (RSVP) and have continued involvement from volunteers in the
community that assist the staff with engaging all of our client’s in daily activities.
- Alzheimer Care and Activities: Our staff receives training and are certified as
“Dementia Specialist” as well as attend ongoing training to remain up-to-date on
new ways to care for the dementia population. We provide daily orientation,
continued verbal reminders on daily routine and surroundings, and most
importantly, maintain their safety with exit seeking, verbal, and physical
behaviors.
- In-house Medical Director: Our medical director is Dr. Scott Evans,
specializing in geriatrics and internal medicine. Dr. Evans is on-call for our
facility for emergency situations as well as being available to see our clients if
requested by our families. In the past, Dr. Evans visited our facility and provided
memory screenings to those who requested the service free of charge. Each
client as the right to continue seeing their current physician. Aging Gracefully
has established relationships with most of the physicians in the tri-county area.
Upon admission, the responsible party signs a release form to communicate with
the client’s primary physician so the registered nurse can telephone the
physician if a problem arises. If a physician is contacted for any reason, the

- In-house Therapists (Physical, Occupational, and Speech): Our Physical
and Occupational Therapy is provided by TX Team Rehab. The registered
nurse monitors in changes of the clients for decreased mobility, shuffling gait,
difficult transfers, frequent falls, decreased independence with ADL’s, etc and
makes a referral to the primary care physician when needed for rehab services.
All responsible parties are notified prior to receiving an order. [Click here to learn
more about TX Team.] Speech therapy is provided by Palmetto Speech and
Language Pathology. The registered nurse monitors clients for difficulty with
swallowing, coughing while drinking liquids, difficulty with feeding self, etc and
makes a referral to the primary care physician when needed for speech services.

- In-house Podiatrist (visits every 9 weeks): Dr. Robert Kish schedules a day
to come to the facility every 9 weeks. Medicare usually pays 100% of this
service. His service includes trimming the toenails, thinning, checking the
circulation in the lower extremities, etc. Dr. Kish also writes prescriptions to
diabetics yearly for new diabetic shoes that are also covered by medicare. If you

the front desk.
- Beauty Salon/ Barber Shop: Our beautician is Betty Hodge. Our beauty
salon/barber shop is open every Friday morning. She provides services for the
hair including shampoo, haircut, perm, color, braids, etc. You may pay Mrs. Betty in
cash or you can make checks payable to Betty Hodge.
- Fitness Program: Aging Gracefully provides daily exercise to the client’s in
hopes to maintain their physical wellbeing. Each day we participate in the
“walking club” where the walks as many times as they can around the loop of the
building. The client with the most laps at the end of the month wins a prize. In
addition the walking club, the staff also engages the clients in chair aerobics,
parachute, kickball, stretching, basketball, bowling, air hockey, etc.
- Library: A range of large print books, magazines, readers digest, and movies
are located on the book shelf in the lounge area for the client’s enjoyment. The
Dorchester County Bookmobile also visits every 3 weeks so that the client has
the opportunity to go on the bus and check out items from the library that they
interested in reading.
