Aging Gracefully offers four different levels of care provided by our well trained staff. All prices are based on Level of Care.

Prices include: Morning snacks, Catered lunches, Afternoon snacks and  daily activities.

Level I:

Daily medication monitoring
Hands-on care by our friendly, well-trained staff Availability of our Licensed Nurse

Level II:

Daily medication monitoring
Hands-on care by our friendly, well-trained staff
Availability of our Licensed Nurse
Assistance to and from meals and activities
Monitoring of individual mealtime needs
Coaching on toileting

Level III:

Daily medication monitoring
Hands-on care by our friendly, well-trained staff
Availability of our Licensed Nurse
Hands-on assistance with meals, including cutting meats, opening condiments, etc
Support with transfers that require one staff person
Scheduled hands-on toileting

Level IV:

Daily medication monitoring
Hands-on care by our friendly, well-trained staff
Availability of our Licensed Nurse
Close monitoring of participants whereabouts and well-being
Intensive personal care and supervision, including routine toileting
Feeding assistance by prompting participants to eat, or standby assistance to monitor swallowing     problems
Management of chronic incontinence, including  laundry and bathing
